SAN ANTONIOApril 15, 2016 /PRNewswire/ — Atique Orthodontics, P.A. (AOPA) is notifying certain patients about a security incident involving unauthorized access to a computer in its office.  Between February 29th, 2016 and March 30th, 2016, an unauthorized outside party accessed an AOPA office computer that could have led to a server that included patient names, dates of births, social security numbers, home addresses, phone numbers, credit cards and insurance information. There is no indication that any patient information has been misused or compromised.

Atique Orthodontics is a San Antonio-based Orthodontics practice. AOPA immediately closed remote access to its office computers. In response to this incident, APOA is adding additional technical precautions to protect patient information from similar activity in the future.

AOPA is notifying all affected individuals by U.S. mail. In order to prevent any potential misuse of their personal information, AOPA is offering all impacted patients identity theft protection services through ID Experts®, a data breach and recovery services expert. Affected patients will receive, at no cost, complete identity recovery services, 12 months of credit monitoring and a $1,000,000 insurance policy. The deadline to enroll in these services is July 18, 2016.
