Florida Medical Clinic, PA notifies 1,000 patients after Greenway Health error exposed PHI

The following statement was posted by Florida Medical Clinic, PA on their web site. The incident was reported to HHS on May 4 as impacting 1,000 patients.

Please be advised from November 18th, 2015 to January 6th, 2016 your Florida Medical Clinic, PA patient due balance statement (sample statement attached) was accessible to industrial account patients who logged onto the Patient Portal.

THE STATEMENT DID NOT INCLUDE SSN, DOB, CREDIT CARD NUMBERS, OR BANKING INFORMATION.

The statement was limited to the following information:

  • Name
  • Date of service
  • Provider
  • Description of procedure
  • Charges due
  • Mailing address

Upon investigation, Florida Medical Clinic, PA discovered a setting was turned on by our Patient Portal vendor Greenway Health. Florida Medical Clinic, PA immediately shut down the setting and the issue is now resolved.  Please know Florida Medical Clinic, PA takes these matters very seriously and has implemented additional protocols with Greenway Health to prevent any further occurrences.

Florida Medical Clinic, PA has no indication that any of your information was negatively impacted; however, in order to protect yourself from potential harm resulting from this incident, you may want to obtain and monitor your credit report.  The Fair Credit Reporting Act guarantees you access to your credit report for free from each of the three (3) nationwide credit reporting companies-Experian, Equifax and TransUnion, every twelve (12) months.

You may request a copy of your credit report by calling 1(877)322-8228 or by accessing the web site at www.AnnualCreditReport.com, or by requesting the report in writing by completing the attached form and mailing it to the address listed at the top of the document (Annual Credit Report Request Service, P.O. Box 105281, Atlanta, GA.  30348-5281). You do have the option to request all three reports at once, or to order one report at a time.  By requesting the reports separately, you can monitor your credit at intervals, to help in the detection of any unauthorized activity.

Again, Florida Medical Clinic, PA has no reason to believe your information has been used inappropriately.
