Inland Empire Health Plan notifies Children’s Eyewear Sight customers of data theft (Update1)

Inland Empire Health Plan (IEHP) is notifying plan members of a breach at a local provider’s facility.

On October 28, a desktop computer and other items were reportedly stolen from Children’s Eyewear Sight in Rancho Cucamonga, CA. A file on the computer included plan members’ name, date of birth, gender, address and contact phone number, email address, IEHP Member ID number, appointment date, date of purchase, and the name of the doctor who provided the optical prescription. Some of those affected were plan members’ dependent children.

The breach was discovered by IEHP on November 14th, and IEHP’s notification letter indicates that a suspect was apprehended.

As part of its remediation, IEHP offered members the option of applying a confidentiality alert to the electronic record maintained by IEHP.

Update: According to a notice on IEHP’s web site, the breach affected 1,030 IEHP members.
