Lost garda documents fell from back of bike

Garda documents fell off the back of a motorcycle when it hit a speed bump and were never recovered, according to records.
The incident was one of more than 40 data-protection breaches involving sensitive information held by the Department of Justice and Equality that were recorded last year.
