The following breaches were reported on HHS’s breach tool, but no media coverage or statements have been found to provide additional details. If you have additional information on any of these, please use the Comments section to let me know.

Thomas Cristello, Chiropractor PC in New York reported that 914 patients had information on a portable electronic device lost on July 21.

The Longstreet Clinic, P. C. in Georgia reported that 720 patients were affected by an incident on June 26th involving “improper disposal.”

Christiana Care Health System in Wilmington, Delaware notified HHS that VARO Healthcare, a business associate handling accounts receivable, had a reportable breach on September 3. According to HHS’s public breach tool, 1,667 patients were affected by a breach involving paper records. Christiana did not respond to an inquiry from PHIprivacy.net asking for details of the breach.

Cone Health Medical Group in North Carolina reported that 1,872 patients were affected by a breach on September 8 coded as Unauthorized Access/Disclosure” of paper records.
