As seen on the web site of Dr. Richard Boehm:

As required by law, this notice is to inform our patients that some of your confidential personal information may have been compromised. It has come to our attention that an employee may have printed documents containing some of our patients’ personal information from our computer files and removed these documents from our office. This employee has been dismissed. Dr. Boehm takes your privacy and his duty to protect it very seriously. Local police, the federal government, and the state dental board have all been notified of the incident and formal complaints have been filed. Dr. Boehm intends to see that this former employee is prosecuted to the fullest extent of all federal and state laws, and disciplined by the state dental board.

Please contact Dr. Boehm or his office manager if you have any questions or concerns,

Dr. Richard Boehm

According to their submission to HHS, 500 patients were affected. Unfortunately, they do not seem to publicly disclose what kind of PHI was involved or what the employee’s motivation might have been.
